Support Coordination is a Capacity Building support that assists participants to understand, implement, and maximise their NDIS plan. It plays a critical role in turning NDIS plans into practical, coordinated supports that help participants work towards their goals with clarity and confidence.


At OSCS, we work closely with participants to reduce the complexity of the NDIS, strengthen choice and control, and ensure supports are aligned with individual needs, priorities, and preferences. We take a collaborative and proactive approach, recognising that each participant’s journey is unique.

What does a Support Coordinator do?

  • Understand their NDIS plan, funding categories, and approved supports
  • Clarify goals and priorities and translate them into practical actions
  • Identify and connect with suitable NDIS and community service providers
  • Coordinate multiple supports to ensure services work together effectively
  • Assist with service agreements, funding use, and budgeting to support effective plan implementation
  • Build skills to manage supports independently and make informed choices
  • Strengthen confidence, self-advocacy, and decision-making
  • Respond to challenges, changes, or barriers as they arise


Support Coordinators act as a central point of contact, supporting communication between participants, providers, and other stakeholders to ensure supports are consistent, responsive, and well-coordinated.

Support Coordination helps participants move from planning to action. By ensuring the right supports are in place and working effectively together, participants are better positioned to make progress toward their NDIS goals.

Monitoring progress and continuous improvement

We actively monitor how supports are working and regularly review progress against individual NDIS goals. This includes:

  • Checking that services are meeting expectations and participant needs
  • Reviewing outcomes and identifying areas for improvement
  • Adjusting supports where required to remain aligned with goals
  • Maintaining open communication with participants and providers


This ongoing review process ensures supports remain relevant, effective, and responsive, while promoting continuous improvement and participant satisfaction. We walk alongside participants at every stage of their NDIS journey — supporting them to navigate the system, build capacity, and work towards the life they choose.
