Our HCP Supports
At Orient Support and Care Services (OSCS), we provide support to individuals who access Home Care Package (HCP) funding through an approved aged care provider. We are not an approved Home Care Package provider; instead, we deliver services as a subcontracted or third-party service provider, working in partnership with approved providers to support clients to live safely, comfortably, and independently in their own homes and communities.
We recognise that every person has unique needs, routines, health considerations, and personal preferences. Our approach is person-centred, culturally responsive, and focused on dignity and choice. With the support of experienced care staff, OSCS delivers tailored services that promote independence, wellbeing, social connection, and meaningful participation, in line with each client’s care plan. Whether support is required with daily living tasks, social engagement, mobility, or other agreed supports, our team works collaboratively with the approved provider to deliver reliable and compassionate care.
Our Home Care Package supports cover a wide range of essential care services that help participants maintain independence and live more confidently at home, our HCP supports include:-
Personal Care
We provide respectful and person-centred personal care support to assist clients with daily self-care tasks such as showering, grooming, dressing, and toileting. Our approach focuses on maintaining dignity, privacy, and independence while supporting individual routines and preferences, in line with the client’s care plan and instructions from the approved Home Care Package provider.
Domestic Assistance
Our domestic assistance services help clients maintain a safe, clean, and comfortable home environment. Support may include light household tasks such as cleaning, laundry, dishwashing, and changing bed linen, enabling clients to continue living independently in their own homes.
Social Support and Community Access
We support clients to stay socially connected and actively involved in their community. This may include companionship, attending social or cultural activities, shopping, appointments, or community events. Our services aim to reduce isolation, promote wellbeing, and support meaningful participation.
Assistance with Daily Living
We assist clients with everyday activities that support independent living, such as meal preparation, medication prompts (non-clinical), and support with daily routines. Services are delivered in accordance with the individual’s care plan and designed to enhance confidence and quality of life.
Transport Support
We provide transport assistance to help clients attend medical appointments, social outings, community activities, and essential errands. This support enables clients to remain engaged with their community and access services safely and reliably.
Wellbeing and Lifestyle Support
Our wellbeing and lifestyle supports focus on maintaining physical, emotional, and social wellbeing. This may include gentle activities, routine support, encouragement with healthy habits, and engagement in meaningful daily activities that align with the client’s interests and goals.
Support for CALD and Diverse Communities
We deliver culturally responsive supports that respect each person’s cultural background, language, values, and lifestyle. Our team works sensitively with individuals and families from diverse communities to ensure services are inclusive, respectful, and aligned with cultural needs.
Home Safety and Maintenance Support
We support clients to maintain a safe and functional home environment by assisting with minor home safety and maintenance tasks. This may include identifying safety risks, arranging or supporting minor repairs, changing light bulbs, installing basic safety aids, and supporting home organisation to reduce hazards. All supports are delivered in line with the client’s care plan and through their approved Home Care Package provider to promote safety, independence, and peace of mind.
